Referral Pathways – Core Arts Membership

There are a few different routes to becoming a member at Core Arts, and we want to make sure we help you or your client find the right pathway.

Commissioned partnership pathways

Option 1:

Recovery Alliance – ICB (Core Arts & ELFT) 

This pathway works in partnership with ELFT to support Hackney residents with severe and enduring needs as part of their current care plan or discharge from hospital. The aim is to reduce re-admission and use of NHS services. Referrals are prioritised with flexible access to membership.

Members attending via Recovery Alliance are eligible to choose sessions from our entire timetable.

Membership is 3-to-6-months for up to 2–3 sessions per week.

  • Residents of Hackney who are inpatients or have recently been discharged from City and Hackney Centrefor Mental Health
  • Residents of Hackney under community mental health teams such as Neighbourhood Recovery Service, EQUIP, ABI & some residents under Home Treatment Team
  • Ward clinicians
  • Occupational Therapists
  • Nurses
  • Psychiatrists
  • Care Coordinators

Refer via our Recovery Alliance Referral.

Target engagement of this pathway is (but not restricted to):

  • Afro-Caribbean Heritage (ACH) Males/Cluster 12 & 13 (over-represented in secondary/wards)
  • or Marginalised/socially isolated or underserved persons.

Option 2:

City & Hackney Wellbeing Network

The City and Hackney Wellbeing Network is a partnership of Hackney based voluntary sector specialist mental health providers.

Those referred via this route will be offered a selection of our courses as well as a range of sessions across the network such as creative arts groups, physical health sessions, complimentary therapies and social groups.

Access to the network is for one year.

Adult residents of City & Hackney with a range of mental health needs and varying levels of support (please also check our Recovery Alliance pathway to see if this is more appropriate for your client).

  • Self-referrals are welcome
  • Any person working with client can apply on their behalf
  • Mental health coach from Core Arts or other Wellbeing Network provider
  • Access to Creative Arts education/ courses
  • Access to Core Sport sessions
  • Shared care options
  • Make a referral via the Wellbeing Network website https://chwellbeingnetwork.london/
  • Please select Core Arts as your preferred Network organisation for dedicated mental health coach/support
  • Your referral will be screened and allocated by Mind CHWF who manages the Wellbeing Network

Individualised funding pathways

Option 3:

Funded placements: Personal budget or direct payments – All London Boroughs

Personal budgets are individualised funds for people who have identified Core Arts as part of their ongoing care and support plan.
Members attending via a personal budget will be eligible to choose sessions from our entire timetable. Therefore, create a personalised programme to suit their interests and creative goals. Minimum membership is 6 months and can attend for up to 3 sessions per week.

  • Adults from any London borough who have primary support needs related to mental health and are eligible for a personal budget (has long term needs under the Care Act and/ or S117 aftercare status)

  • Occupational Therapists
  • Psychiatrists
  • Care Coordinators
  • Social Workers

  • Apply for funding according to your local authority’s/clinical team guidelines

  • Inpatients in John Howard Centre and Wolfson House can be supported to apply for funds via their clinical teams for an individualised budget.

Option 4:

Personal Health Budget (PHB) for City and Hackney, Tower Hamlets and Newham (ELFT)

Personal Health Budgets are personalised funding for supporting people to meet a timebound mental health recovery goal (www.corearts.co.uk/phb-brokerage-service). Currently, they can be used to purchase Core Arts membership for 13 weeks (3 months) at the cost of £585.
A member joining us via PHB would be eligible to choose one session from our entire timetable per wee.

  • An adult (18+)
  • Receiving mental health support from East London Foundation Trust
  • A resident of City and Hackney, Newham or Tower Hamlets

ELFT Mental Health Clinicians (City and Hackney, Newham or Tower Hamlets)

This is a two-part referral:
